LVN or LPN - Full Time and PRN

Join YesCare as a Licensed Practical Nurse (LPN) and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program.

Currently seeking PRN for day/Night shifts.

Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans.

Provide safe, effective nursing care for multiple patients within the facility’s healthcare program.

Perform delegated nursing tasks consistent with educational preparation and within the scope of practice.

Oversee onsite and offsite patient scheduling in collaboration with the care team.

Ensure follow‑up care for patients, including telemedicine appointments and consult processes in accordance with policy and contract requirements.

Participate in Quality Improvement initiatives and Patient Care Committee meetings.

Coordinate with Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care.

Perform additional nursing duties as assigned to support patient outcomes and organizational goals.

Graduate from an accredited Licensed Practical Nursing program.

Current licensure as a Practical Nurse (LPN) in the state of employment.

Familiarity with state laws and regulations governing LPN practice.

Ability to pass a background check, security clearance, and drug screening.
