What you will love doing Provide skilled nursing care in a patient’s home as directed by the medical director and/or attending physician.

Manage and facilitate resource applications to meet patient needs.

Support chart review and patient phone calls from home when there are minimal to no after‑hours needs during the shift.

Observe and monitor patient conditions and perform comprehensive assessments to develop an individualized care plan and adjust as needs change.

Evaluate the treatment and medical condition of the patient according to the plan of care.

Determine if the level of care being provided meets the patients’ needs and facilitate level of care changes, including the management of after‑hours routine, general inpatient, respite, and continuous care.

Administer medication as prescribed by the physician and collaborate on required updates to the plan of care to achieve comfort and symptom control.

Prioritize visits for high‑risk patients.

Manage multi‑disciplinary care as applicable while promoting continuity of care with collaborative determinations on level of care provisions, additional resource need identification, and schedule management.

Complete chart reviews and answer patient calls to support patient care and regulatory compliance.

Patience when working with patients, families, physicians, and coworkers.

Requirements Registered Nurse with current license in the state of employment.

2 years of hospice experience preferred.

Previous experience with clinical management, hospice case management, or other relevant clinical experience.

Valid driver’s license and auto insurance in your name as a driver.
