Licensed Vocational Nurse (LVN) – Home Health Focus Home Health | Redwood City, CA & Surrounding Peninsula Communities

Focus Home Health is growing our clinical team! We're seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in the comfort of their homes. This opportunity servces patients throughout the Peninsula, with a coverage area extending from North Palo Alto to San Mateo, including Redwood City, Menlo Park, San Carlos, and surrounding communities.

Provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN).

Follow the established plan of care and perform ordered preventative, therapeutic, rehabilitative, comfort, and pain-management interventions within the LVN scope of practice.

Monitor patients for changes in condition and promptly communicate significant findings to the RN, physician, and other appropriate members of the care team.

Perform treatments and procedures such as medication administration, injections, wound care, vital-sign monitoring, and other skilled nursing services as ordered.

Educate patients and caregivers regarding medications, treatments, disease management, safety, and the plan of care.

Assess the home environment for factors that may affect patient safety and make appropriate recommendations to the care team.

Obtain and document verbal orders from healthcare providers as appropriate and within the LVN scope of practice.

Help patients achieve greater independence, remain safely at home, and reduce the risk of avoidable hospitalization.

We’re looking for an LVN who combines strong clinical skills with compassion and a genuine commitment to patient-centered care. You understand that home health is about more than completing a visit—it’s about getting to know your patients, recognizing changes in their condition, educating families and caregivers, and helping each patient achieve the best possible outcome.

Graduate of an accredited nursing program.

Current California Licensed Vocational Nurse (LVN) license in good standing.

At least one year of clinical nursing experience preferred.

Home health experience is strongly preferred.

Ability to work independently while collaborating effectively with the interdisciplinary care team.

Ability and willingness to travel throughout the assigned service territory, including North Palo Alto, Redwood City, San Mateo, and surrounding communities, to provide in-home patient care.
