Transform bedside care with AI. As Circadia's on-site NP, you'll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions-reducing preventable readmissions. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.
This is a full-time, embedded NP role requiring Monday-Friday, on-site presence at our partner facility, in Fullerton, CA.
Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; coordinate with facility providers.
Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
Data into action : Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
Quality & product feedback : Surface field insights to improve workflows and tech integration in a clinical setting
Visits: Target 30 patient visits/day
Adoption & impact: Demonstrated workflow adherence at facilities; Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.

• 2+ years NP experience with complex adult populations; comfortable traveling locally.
• English required

Google Suite, multiple EMRs, Slack, AI transcription/tools.
Spanish is preferred.
$As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $130,000 - $160,000 . Annual salary is only one part of an employee's total compensation package at Circadia Health. 100% company-paid medical, dental, and vision coverage
