LPN Compensation (1099):
Hi-Tech / Complex Care Visits: $35.00
Actify Home Care is clinician-owned by a Physical Therapist and clinically led by a veteran Director of Nursing—each with over 20 years of home health experience. We understand the physical and clinical demands of field-based nursing because we’ve been in your shoes. Clinician-to-Clinician Support: Work with a veteran DON who understands the realities of field nursing and provides expert guidance on complex cases.
We pay set flat rates per completed visit, to respect your time and travel.
Reliable Direct Deposit: Convenient electronic payments every two weeks, processed automatically based on your completed Kinnser visit logs—no invoice tracking required.
What You’ll Do as an LPN:
Patient Care: Provide high-quality, one-on-one therapy to geriatric patients in their homes.
Care Plans: Implement and follow treatment plans established by the supervising RN.
RN Collaboration : Coordinate and communicate patient status regularly with the supervising RN.
Active Florida Registered Licensed Practical Nurse (LPN) License
Software: Kinnser/WellSky experience is a major plus!
