Caseworker (Full Time)

Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays.

Social Worker Outpatient - Licensed
Perform evaluations and services to patients of various age groups, collect data and render a professional opinion about treatment and care.

Perform vocational rehabilitation assessment in a timely manner; dictate interdisciplinary report to clinical record within the required time frames.

Initiate and discharge documents within required timeframe.

Document therapy notes on the clinical record.

Identify and document patient care needs.

Work closely with the interdisciplinary team to assure appropriate patient care; Provide patient/family emotional support and education.

Meet in-service requirements including mandatory education.

read, understand, and adhere to all Physician Services policies and procedures.

Post Graduate degree.

A Clinical Mental Health Counselor license or Independent Clinical Social Worker license required in accordance with state regulations.
