Registered Nurse (RN) - PRN - Home Health Care - PRN or Weekend

Hospice of Marion, a Part of Empath Health , is seeking an RN On Call (PRN) to join our team. Visits patients and provides clinical, physical, educational and emotional support after-hours in in the patient home, long term care facility, assisted living facility in Marion County .
Schedule: Evening and Overnight shifts when needed and available; 2 weekend shifts per month (12-hour shifts)

Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines.
Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational inservice or seminar as assigned.
Provides education to IDT members regarding nursing aspects of patient/family care.
Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.

Minimum of one year related experience and/or training; Valid Florida license as a Registered Nurse
Prefer one (1) year experience in an acute or post acute care setting
