Home Health Physical Therapist
This is a 13-week contract position.
Day shift - 5x8 Hour schedule (08:00 - 17:00)
We are seeking an experienced Home Health Physical Therapist to provide high-quality care to patients in a variety of clinical settings, including med/surg, cardiac, and ortho cases. The therapist will conduct patient assessments, develop and implement individualized treatment plans, and ensure proper documentation of patient progress.
Responsibilities
Conduct thorough assessments of patients in their homes.
Develop comprehensive treatment plans to enhance patient mobility and function.
Implement effective therapeutic interventions utilizing adaptive devices and modalities.
Educate patients and families on home exercise programs and safety protocols.
Document patient progress and outcomes in compliance with regulations.
Collaborate with interdisciplinary teams to optimize patient care.
Required Experience / Certifications / Licensure
Minimum of 1 year of experience in home health care.
Valid state licensure as a Physical Therapist is mandatory.
Current BLS certification required.
OASIS training preferred.
Experience with Medicare, Medicare Advantage, and commercial insurance documentation.
