LVN & Therapist

Cherrelyn Healthcare Center, part of Stellar Living’s mission-driven network, is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality, person-centered care to our residents. In this role, you will deliver direct nursing care such as medication administration, treatments, wound care, and monitoring vital signs while supporting residents’ physical, emotional, and social well-being. You will collaborate closely with RNs, physicians, therapists, CNAs, and families to carry out individualized care plans, document assessments and interventions accurately in our electronic medical record system, and promptly report changes in condition. As an LPN, you will help coordinate care transitions, support discharge planning, and provide clear, respectful communication to residents and their families. You will promote a safe environment by following infection control standards, participating in quality improvement initiatives, and assisting with staff education and mentoring newer team members.
