Nursing Home Advanced Practice Provider

Job description

An exciting opportunity for an Advanced Practice Provider (APP) to practice autonomously while collaborating closely with private practice, critical care and pulmonary-trained physicians. The Advanced Practitioner will collaborate with all members of the multidisciplinary care team to deliver high-quality care across multiple settings, including long-term care and skilled nursing facilities (LTC/SNF), private practice office, medical centers and telehealth. This role offers a high degree of professional autonomy, supported by physician oversight and availability. Both Nurse Practitioners and Physician Assistants are encouraged to apply.
Core responsibilities may include:

Multidisciplinary rounding;
Performing history and physicals/daily progress note documentation;
Ventilator management;
Clinical response to change in patient condition;
Family discussions and meetings;
Bedside procedures to include central line/hemodialysis catheter insertion and arterial lines.

Requirements :

  • Graduate of an accredited NP/PA training program;
  • Master's degree;
  • Active Pennsylvania State License; and
  • 1-3 years of experience preferred, ideally in pulmonary care; however, highly qualified new graduates will be considered.
