Responsibilities Provide expert symptom management, crisis intervention, and comfort-focused care to hospice patients in their homes.

Develop, implement, and update individualized care plans in collaboration with an interdisciplinary team; educate patients and families on disease processes, treatments, and end-of-life care.

Document care activities accurately in the patient record and communicate changes to the care team; participate in care conferences and care coordination activities.

Travel to patient residences and to affiliated facilities or agencies as needed within the service area; Maintain professional standards, ensure patient safety, and comply with applicable regulations and agency policies.

Requirements Active Virginia RN license

1+ year hospice experience required

Ability to travel within the hospice service area; overnight travel may be necessary for continuing education
