Ambulatory Care Pharmacist - Primary Care - Full Time

We are seeking qualified Pharmacist to service our patients though out the Charleston area. Pharmacists play a vital role in ensuring the safe and effective use of medications to improve patient outcomes. Our pharmacist conducts virtual patient visits from the comfort of their own homes .This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
The following shift(s) are available:

Your Health is currently looking for a Pharmacist to join our growing primary care family. A successful Pharmacist will be able to perform these essential duties and responsibilities. Provide medication therapy management (MTM) services, including comprehensive medication reviews, medication reconciliation, and adherence assessments.
Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans for high-risk patients with complex medical needs.
Collaborate with internal and external resources to facilitate and ensure seamless operations.
Meet staff efficiency expectations by performing consultations, medication reviews, and other clinical pharmacy services efficiently and effectively.
Maintain accurate and up-to-date documentation of patient encounters, medication therapy interventions, and clinical recommendations in electronic health records (EHR) systems.
Generate reports, analyze medication-related data, and participate in quality improvement initiatives to enhance patient care outcomes and medication safety.
Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program, required.
Licensed Pharmacist in good standing with the state board of pharmacy.
Clinical pharmacy experience preferred.
Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes. Basic computer knowledge.
Must be able to work flexible hours and travel between offices, facilities, etc.
