Job Summary

The Social Worker 2 will support the Clinical Care Team by managing referrals from primary care providers and addressing social determinants of health (SDOH) needs. This role involves social support navigation, case management, patient follow-up, resource coordination, and community engagement.

Key Responsibilities

Social Support & SDOH Navigation

Provide in-person/remote screenings for social needs (housing, food, clothing, mental health, etc.)
Maintain SDOH resource lists with eligibility, referral processes, and contacts
Coordinate with nurses, providers, and fellow social workers
Connect patients to appropriate community resources
Case Management

Support hospital discharge planning and follow-up
Obtain patient records from hospitals and specialists
Assist in securing medical equipment
Conduct care plan follow-ups and identify patients lost to care
Specialty Referral Support (as needed)

Schedule and track specialist/imaging referrals
Retrieve records from specialists
Maintain specialty referral resource lists
Medication Access Assistance

Guide patients to low-cost prescription programs
Assist with applications (CoverRx, RxOutreach, PAPs)
Serve as liaison between patient and drug programs
Additional Duties

Support regional primary care initiatives
Document all interactions and update patient records
Provide or connect patients to educational resources
Attend clinical meetings, provider meetings, health councils, and community events
Identify and escalate patient barriers to care
