Current license to practice Audiology in the State of Ohio issued by the Ohio Board of Speech Pathology and Audiology. OR License pending completion of CFY training as stated by the Ohio Board of Speech Pathology and Audiology.
Current Basic Life Support for Health Care Provider (CPR) certification through the American Heart Association
