We are seeking an experienced MDS Coordinator - RN to join our well-established, not-for-profit Continuing Care Retirement Community (CCRC) in Fort Wayne, Indiana. Lead and oversee the MDS process for a large, skilled nursing census

Drive care planning and collaborate with interdisciplinary teams

Monitor case mix index (CMI) and support reimbursement optimization

Serve as a resource and mentor for clinical team members

Maintain survey readiness and regulatory compliance

Active RN license in the state of Indiana

Proven experience as an MDS Coordinator in a skilled nursing or CCRC setting

Strong understanding of Medicare/Medicaid, PDPM, and reimbursement processes
