Home Health Physical TherapistThe Home Health Physical Therapist provides high-quality, patient-centered physical therapy services to individuals in their homes. In this role you will create personalized therapy plans, implement therapeutic interventions, and work closely with patients and their families to achieve optimal functional outcomes.Schedule: PRN, WeekdaysPay: $75 - $175 Per VisitPrimary Responsibilities and Duties:Conduct thorough evaluations of patients' physical abilities, limitations, and needs to develop individualized therapy plans.Develop and implement effective treatment programs to improve patients' mobility, strength, flexibility, and overall functional performance.Utilize therapeutic exercises, manual therapy techniques, modalities, and assistive devices to address specific physical impairments and goals.Monitor and document patient progress, responses to therapy, and any changes in condition providing detailed reports to the supervising physicians and other healthcare team members.Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated approach to patient care.Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.Participate in the development of care plans and case conferences with the healthcare team to optimize patient outcomes.Tay currently has advancements in physical therapy practices and participates in ongoing professional development.Advanced Home Care & Hospice of Northern Idaho is looking for a physical therapist to join our amazing Lewiston, Idaho team. Home health provides flexible schedules and competitive pay. The position requires the ability to problem solve, be organized, detail-oriented, and possess practical communication skills both orally and in writing. Previous home health or hospice experience is preferred but not required.Valid Physical Therapy license in the state where work is performed.2 or more years of physical therapy experience, preferably in a home health or similar setting.Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.Ability to lift and transfer patients, with or without assistance, up to 50lbs.Strength to handle therapeutic equipment and assist patients with mobility and physical activities.Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling therapeutic tools.Ability to respond swiftly and efficiently to emergency situations.Medical Specialty:Home HealthLicense/Certification:Physical Therapy License (required)CPR Certification (preferred)Ability to Commute:Lewiston, ID 83501 (required)Ability to Relocate:Relocate before starting work (required)
