Job Description Physical Therapist (PT) – Home Health Location: Phoenix, AZ – West Valley (territory west of I-17; formal map pending)
Employment Type: Direct Hire / Permanent
Compensation: $140,000 annual base + overage bonus + $2,000 sign-on bonus
About the Opportunity A growing home health agency serving the West Valley of Phoenix is seeking a licensed Physical Therapist to join their team on a permanent, direct-hire basis. This is an immediate need — the agency is actively interviewing and ready to move quickly for the right candidate.
Position Summary The Home Health Physical Therapist will provide skilled, in-home physical therapy services to patients throughout the West Valley territory (area west of I-17), delivering individualized treatment plans that support patient recovery, mobility, and independence within the home setting.
Key Responsibilities
Conduct comprehensive in-home evaluations and develop individualized treatment plans
Provide skilled PT interventions in accordance with physician orders and plan of care
Document visits, progress notes, and outcomes in compliance with home health regulatory and payer requirements (Medicare/Medicaid guidelines)
Collaborate with interdisciplinary care team members (nursing, OT, SLP, case management) to coordinate patient care
Educate patients and caregivers on home exercise programs, safety, and self-management strategies
Maintain accurate, timely clinical documentation using agency EMR system
Manage a home visit caseload within an assigned territory efficiently and independently
Requirements
Current, unrestricted Arizona PT license (or eligibility to obtain)
Home Health experience required
Valid driver's license, reliable transportation, and willingness to travel within the West Valley territory
Strong clinical, organizational, and communication skills
Ability to work independently in a home-based care setting
Compensation & Benefits
Base Salary: $140,000/year
Overage Bonus: Available for productivity above target visit volume
Sign-On Bonus: $2,000
Full-time, permanent direct-hire position

Immediate start desired
