Job Description Registered Nurse - Home Health

Provides skilled nursing care to patients in their homes, performing assessments, treatments and care coordination across an assigned caseload. Develops and updates the plan of care in collaboration with the physician and interdisciplinary team. Graduate of an accredited school of nursing: Home health, community or acute care nursing experience: Registered Nurse licensure in the state of practice: Develop, implement and revise the plan of care in collaboration with the physician.

Perform skilled nursing interventions, including wound care, medication management and IV therapy.
Coordinate care with therapy, social work, home health aides and community resources.
