Physician’s Nurse - Full time

Our Community Care Internal Medicine Niskayuna office is looking for a full-time an RN to join their team! This is a full-time position. Monday-Friday, 8:00AM-4:30PM.

**This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Document verbal conversations with patients when appropriate.

Document information into patient record in an accurate, legible and timely manner.

Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.

Support care management activities including coordination of patient needs, follow-up and connection to resources.

Assist with transitions of care - coordinating post-discharge follow-up, medication reconciliation and communication with patients and care teams.

Prepare patient for visit as appropriate, based on office/provider requirements.

Compensation: $32.00 - $45.50 per hour

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Valid NYS RN license.

Previous experience in an RN role.

Strong computer skills.

Knowledge of basic nursing principles and protocols.

Knowledge of medical chart reporting/records.

Skill in appropriate assessment and assistance techniques.
