Inpatient Unit Licensed Practical Nurse
Mercy Health Home Care and Hospice by Compassus
Schedule: 3pm-11pm during the week - every other weekend and holiday
This position may work both in-patient and community hospice.
At Mercy Health Home Care and Hospice (Hospice of the Valley) by Compassus, the Inpatient Unit Licensed Practical Nurse is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. The Inpatient Unit Licensed Practical Nurse participates in directed care delivery including functional and focused nursing tasks as assigned by the Registered Nurse and provides ADL and other support as directed. The Inpatient Unit Licensed Practical Nurse does not function as a Case Manager but as part of a nursing team including a Registered Nurse.
Submits appropriate documentation and paperwork to facilities at the completion of patient visits.
Attends and participates in staff meetings, educational programs and community events, as requested.
Adheres to standards of nursing practice and maintains knowledge of current clinical practice for terminally ill patients.
Participates in quality performance improvement projects and clinical record reviews as directed.
Communicates effectively with RNCM, DCS and physicians (as applicable).
Provides nursing services as directed by the RN.
Gathers clinical data and provides to RN via documentation and communication in order to support comprehensive assessment and plan of care management.
Care of the patient
Medications, including expected effect, side effects, and when to call hospice
Wound care
High school diploma or GED required.
Minimum of two (2) years of full time experience as an LPN required.
Experience in skilled nursing, preferably in hospice, home health care, oncology or emergency care highly preferred.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
Certifications, Licenses, and Registrations
~ Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment required.
