Graduate of an accredited vocational school of nursing

Current LPN license (or equivalent)

Previous experience in Long Term Care, Home Health, acute care nursing, or Hospice (preferred)

Computer literacy

Perform patient visits under the supervision of an RN Case Manager

Provide patient care as delegated by the RN

Monitor nursing care provided by other personnel

Assist with data collection and plan of care development

Adhere to clinical nursing practice standards and the Nurse Practice Act
