The Opportunity
As an LPN you will provide nonclinical support, educating clients and caregivers in areas including health maintenance and preventative measures, assisting in healthcare management navigation, and accessing community support. Responsibilities

Complete intake and annual assessment and admission visits with nursing.

Conduct monthly in‑home visits, provide caregiver education, and assess psychosocial and environmental changes.

Perform semi‑annual visits to review and update existing plan of care and goals with nursing, assessing qualified setting and reviewing caregiver communication tools.

Assist with obtaining information and accessing other healthcare and community services.

Participate in multidisciplinary care plan creation, review, and discharge planning.

Address concerns regarding environment and psychosocial status changes.

Participate in multidisciplinary team meetings and collaborate to address client needs.

Complete all required documentation per regulation and communicate professionally with client, PCP, and other healthcare providers when necessary.

Perform other duties within scope of practice as assigned by management.

Qualifications

Licensed by the Massachusetts Board of Registration in Nursing.

At least two years of recent experience in direct care of elders or adults with disabilities.

Excellent interpersonal and communication skills.

Valid driver’s license with proof of insurance.

Bilingual in English and a secondary language preferred.

Relevant experience in healthcare, home health care, case management, or sales preferred.

Work Environment and Physical Requirements

Office‑based work in a clean, well‑lit environment with fluctuating temperatures.

Client‑facing field staff require frequent travel to client homes or community settings; ability to drive safely; work in client homes may involve varying temperatures, odors, allergens, pets, and other environmental factors.

Substantial periods of repetitive work using a computer, monitor, keyboard, and mouse.

Lifting and carrying equipment and supplies weighing up to 35 pounds; pushing and pulling up to 35 pounds; walking and standing; frequent sitting more than 75% of the workday; negotiating stairs; visual acuity and manual dexterity to operate equipment.
