Heartland Home Health, a Gentiva company, is seeking a Full‑Time Physical Therapist to join our Midland, MI team covering the Saginaw territory. This position delivers personalized, meaningful care in the comfort of patients’ homes.

What You’ll Do as a Home Health Physical Therapist

Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with the referring physician.

Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.

Adjust treatment plans and techniques as needed to achieve optimal outcomes.

Collaborate with the interdisciplinary home‑health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.

Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.

Document all visits, assessments, and progress notes accurately and in a timely manner.

Support patients in maximizing independence and improving quality of life regardless of where they live.

Qualifications – What You’ll Bring

Master’s degree in Physical Therapy from an accredited program.

Current PT license in Michigan (or eligibility to obtain one).

CPR certification required.

Strong communication, assessment, and problem‑solving skills.

Ability to work independently in a variety of home environments.

Valid driver’s license, auto insurance, and reliable transportation.

Preferred: 1+ year of experience in hospital, home health, outpatient rehab, in‑home therapy delivery, geriatric, orthopedic, or neurological rehab, post‑acute care, or chronic condition management; experience working as part of a mobile or interdisciplinary care team.
