Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• CHPN certification - Preferred
• Bachelor of Science in Nursing - Preferred

Roles & Responsibilities

• Assess home health and/or hospice patients to identify needs and develop plans of care.
• Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and families on care, safety, medications, and self-care practices.
• Supervise and provide clinical direction to assistive personnel to ensure quality service.
• Assess and educate patients on pain management techniques.
• Provide effective communication and serve as a liaison between patients, families, and interdisciplinary teams.
