Primary Health Care Advanced Practice Nurse

Clinical Care Registered Nurse (CCRN)
Monday to Friday, 8:00 AM - 4:30 PM - on-call/holiday rotations
The Clinical Care Registered Nurse (CCRN) provides direct nursing care to participants, working independently while applying advanced nursing knowledge and skills to manage complex clinical situations. Triaging emergent situations and applying advanced nursing knowledge independently.
Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
Utilizing care plans to provide clinic and home-based direct care.
Making regular home visits for care and treatment as ordered by the PCP, including wound care, lab draws, and nursing assessments.
Bachelor's degree preferred in Science of Nursing; RN Nursing Diploma or Degree from an accredited School of Nursing or University.
A minimum of one year of experience working with the frail, elderly, or long-term care population.
Minimum of one year of home care experience preferred.
One year of wound care experience preferred.
