Rehab Without Walls is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community-based Neurorehabilitation Program in San Francisco, CA. This PRN opportunity offers meaningful flexibility while allowing you to deliver impactful, relationship-driven care in the settings that matter most, such as patients' homes and communities .

An experienced LCSW passionate about providing counseling services that support an individual's physical, mental, cognitive, and emotional adjustment throughout recovery.

A clinician skilled in delivering therapeutic interventions and resource recommendations that help patients achieve personal, career, and independent living goals in the most integrated settings possible.

A team-oriented clinician who enjoys working alongside a diverse group of providers, including Occupational Therapists, Physical Therapists, Speech-Language Pathologists, Music Therapists , and other rehabilitation professionals in the San Francisco area.

PRN Caseload Details:

Sets measurable objectives that are formulated in conjunction with the rehab team

Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems

Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs

Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Master's Degree in Clinical Social Work (LCSW)

Current Licensed Clinical Social Worker Licensure in the state of practice

Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis

Current BLS (Basic Life Support) First Aid and CPR certification required

Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
