Description

American Traveler is seeking a Clinical Care RN for a PACE clinic and home health setting, requiring an active MI RN license and geriatric care experience.

Details

Combined clinic and home-based care setting serving frail, elderly, and long-term care participants

Schedule is Monday through Friday, 8:00am to 4:30pm

On-call and holiday rotation required

Provides direct nursing care including wound care, lab draws, and nurse assessments during home visits

Collaborates with Primary Care Providers and coordinates monitoring of acute and chronic conditions

Requirements

Active MI RN license required

Associate's degree required; Bachelor's degree in Nursing preferred

RN diploma or degree from an accredited School of Nursing or University

Minimum of one year experience working with frail, elderly, or long-term care population

One year of home care experience preferred

One year of wound care experience preferred

Current CPR certification by AHA required, or willingness to obtain

Must have reliable transportation, a valid MI driver's license, and car insurance

Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B

Additional Information

Works independently applying advanced nursing knowledge to manage complex clinical situations

Triages emergent situations and administers treatments, procedures, and medications as prescribed

Maintains a visible presence within the center and communicates regularly with staff, participants, and caregivers

May be required to work beyond normal hours, including weekends and holidays, as necessary
