Flexible remote LPN position

This is a full time, remote position with the occasional need to travel to the New London office. We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail-oriented Licensed Practical Nurse (LPN) to join our Intake Department for 40 hours each week. This person will assist with both Homecare and Hospice referrals.

Tuition reimbursement

Accurately enter patient referral information into the electronic medical record system

Accurately enter all incoming patient care orders and notify appropriate team members as necessary

Navigate multiple electronic referral software portals

Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships

Triage all incoming phone calls

Communicate effectively with patients and patient care givers

Active NH (LPN) Licensed Practical Nurse License

2 years of Health Care experience

Knowledge of medical terminology, familiarity with a medical office setting, and/or triage

Knowledge of Home Health/Hospice

Must be comfortable working in a remote environment

Proof of Flu vaccination
