Registered Nurse- Hospice Health

The Registered Nurse – Home Health position allows you to provide high‑quality, 1:1 patient care in the patient’s home, supporting over 60,000 patients each day across our areas of coverage.

Job Overview As a Registered Nurse Home Health, you will work autonomously while collaborating with an interdisciplinary team to deliver personalized care that aligns with Elara Caring’s vision and HIPAA privacy requirements.

Responsibilities Ensure all activities performed align with Elara Caring’s board of directors, executive team, and Home Health leadership.

Maintain patient and staff privacy and confidentiality pursuant to HIPAA.

Assess assigned home‑health caseload to identify physical, psychosocial, and environmental needs of patients and families.

Assume primary responsibility for the caseload – including assessment, planning, coordinating, implementing, and evaluating the plan of treatment.

Conduct initial and follow‑up nursing evaluations for each visit, gathering data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.

Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physicians, facilities, and other personnel involved in patient care.

Revise the plan of care in response to identified issues and notify the Clinical Supervisor and other team members.

Qualifications Current, unrestricted state RN license.

1+ year clinical nursing experience.

Ability and willingness to travel within the branch/office coverage area; 50% travel expected.

Ability to lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

Associate’s or Bachelor’s degree in Nursing preferred.

Previous Home Health or Hospice experience preferred.

Previous experience with HomeCare HomeBase preferred.

Will report to the Clinical Manager or Clinical Supervisor.
