Job Description
Job Description Your Tailor Made Senior Service is seeking Registered Nurses with prior OASIS/Medicare Certified Visits for home health clients in DeSoto, Cedar Hill, Duncanville and Oak Cliff area.
Responsibilities
To provide quality, comprehensive nursing care in the Patient's home as prescribed by the physician in the plan of care
Ensures quality and safe delivery of home health care services in compliance with all applicable laws, rules, regulations and policies
Performs nursing assessments and develops, implements and evaluates plans of care in conjunction with the physician, patient and caregivers per agency policy and procedure
Evaluates the effectiveness of the plan of care, the patient's response to care and outcomes of care and revises the plan of care as needed
Initiates appropriate preventive and rehabilitative nursing procedures following the physician's plan of care
Documents all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notes
Prepares a written plan for the Home Health Aide to follow, if applicable
Responsible for collaborating with the interdisciplinary team to develop the plan of care for the Patient
Manage nursing treatments and activities to meet the needs of each individual patient
Responsible for evaluating utilization of patient services and patient progress
Review evaluation with the care team, patient, family members, physician, and case manager as needed
Other duties as assigned

Your Tailor Made Senior Service Home Health Nurses are paid by the hour, not by the visit
Nurse will have a designated territory of coverage - Prn or Part Time - Home Health contracts with OASIS/Medicare Certified Visits
Requirements:
OASIS/Medicare Certified Visits Experience
Electronic Charting experience
Developed computer skills
Minimum of 1 year of recent home health nursing experience
Most contracts require some on-call
Driver's License and Vehicle with Current Auto Insurance
American Heart Association BLS
Valid U.S. Driver's License
Problem solving skills
Excellent Communication Skills
Positive, Professional attitude
