LPN - Hourly- Part Time

The Clinical Health Advocate (Licensed Practical Nurse) is an integral part of our care team, owning patient relationships and backing a collaborative care plan that drives meaningful impact in our patients’ lives. The role is part‑time (up to 24hours/week) hybrid, with a balance of clinical and administrative duties scheduled Monday through Friday from 8am‑5pm ET. You will spend time in the field traveling to see patients within Aroostook County or working from an office based in Presque Isle, with some remote work from home possibilities. This role is 50% clinical and 50% administrative.

Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR during patient visits.

Perform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocols.

Collect, process, and prepare specimens for laboratory analysis.

Administer immunizations and injections as authorized by physicians and maintain accurate vaccine records.

When in the field, relay pertinent environmental factors that may impact patients’ well‑being to the care team and conduct physical and social screenings as assigned.

Educate patients on treatment plans, medications, and follow‑up care as directed by providers.

Document patient interactions, clinical data, and care activities accurately in the electronic health record.

Conduct population‑health care gap closures efforts and sprints as required per payer contract.

Ensure proper sterilization and maintenance of medical equipment and follow infection‑control protocols.

Be available for field and urgent deployment within your market region, even on unplanned field days.

Connect patients with various Galileo services and teams, including clinical and administrative functions.

Administrative Responsibilities

Act as a local “face” of Galileo, representing the organization professionally and building relationships with stakeholders.

Own overall chart management for empaneled patients, ensuring charts are medically and demographically prepared for efficient care delivery.

Run the list in partnership with the care team to maintain seamless communication and coordination across patient plans.

Maintain cleanliness, organization, and adequate supplies in exam rooms, mobile units, or office spaces.

Route inbound mail appropriately, sending patient letters or medical equipment when necessary.

Request, follow up, and file medical records with local hospital systems, PCPs and regional health information exchanges (HIEs).

Submit population‑health documents to maximize compliance with payer contracts.

Communicate with patients via text, video or phone for care coordination, scheduling, and reactive needs.

Participate in projects and events as assigned by the Director of Operations.

You are a creative problem solver and a compassionate provider of support. With a service mindset and a strong desire to create meaningful impact, you thrive on building trusting relationships and delivering high‑quality care to improve patients’ lives.

At least 2years clinical experience as a Licensed Practical Nurse.

Experience with phlebotomy (required).

Experience with injections (vaccinations) is preferred.

Reliable access to personal transportation.

Creative problem‑solving skills and impeccable organizational abilities.

Strong communication skills, especially the ability to translate complex ideas into simple language.

The hourly rate is $27/hour.
