Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team. Compensation: Local Pay: $55.00 - $70.00/hour (W-2) Travel Pay Package: Yes Location: Fort Wayne, IN, 46804 Weekly Hour Guarantee: 36 Coverage Areas: Within 30-40 miles radius Shift schedule (days, start & end times): Monday to Friday 08:00 AM to 04:30 PM Key Responsibilities:

Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.
Home health experience preferred.
