Certified Nursing AssistantPH Ops of Texas City - Texas City, TX 77591Position SummaryThe Certified Nursing Assistant is an integral role to the nursing team and has a significant impact on the overall delivery of care to facility customers. The CNA is responsible for providing quality care to customers and supporting the goals for shift objectives to enhance team efficiency and customer satisfaction.Essential Duties and ResponsibilitiesProvide and assist with physical care of patients which will include but not limited to bathing, oral hygiene, grooming, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation and range of motionResponsible for real-time documentation of patient care and observationsCommunicate timely with Licensed Nurse any observed or perceived changes in patient's condition or behaviorAssist in the dining process by preparing customer for meals; serves and collects trays, assists in setting up and feeding residents, serves refreshments. Completes meal assessment and records oral intakes accuratelyResponsible for patient transport to scheduled therapy appointmentsAssists in recreational, motivational and socialization activities of care programResponsible for timely response to call lights and providing appropriate response or serviceResponsible for regular rounding with customers to anticipate and identify needs and respond timely and appropriatelyFully understand all aspects of patient's rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting incidents of evidence of patient abuse or violation of patient's rightsMaintain the comfort, privacy, and dignity of each patient in the delivery of services to them. Encourages growth and independence as care progression warrantsAssists in maintaining the cleanliness and safety of customer room. Assists in maintaining a safe customer environmentAdhere to infection control procedures including but not limited to hand washing and isolation practicesAdhere to Par Scanning procedures to ensure accurate tally of supple usage and availabilityEnsure uniform and personal appearance is clean and professionalMaintain clinical competence through completion of annual continuing education requirementsMay participate in interdisciplinary customer and service improvement initiativesCompetenciesThoroughly completes assignmentsPerforms assigned treatment according to patient plan of careReports timely changes in customer's condition to appropriate partyDemonstrates accurate ADL documentationDemonstrates accurate documentation in real-timeDemonstrates proficient knowledge of equipment in customer care areasTreatments and procedures are performed accurately and with proficient techniqueDemonstrates a proactive approach to complete assigned floor dutiesCultureThis is a high performance environment propelled by collaboration through our Teams of Excellence (T.O.E.) This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility's no-smoking policy.Physical DemandsWhile performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds.Position Type and Expected Hours of WorkThis is a position in a post-acute setting that is open 24 hours a day, 365 days a year.
