Licensed Practical Nurse - Primary Health Care

Overview The Licensed Practical Nurse (LPN) provides direct and indirect nursing care to patients and families in routine nursing situations within the LPN scope of practice. Under the supervision of a registered nurse or licensed provider, the LPN assists in patient care, treatment, and comfort. This role is an integral part of the healthcare team, ensuring quality care in alignment with Emory Healthcare's patient and family-centered care standards.

Responsibilities Provide patient care based on an individualized plan delegated by the Registered Nurse/Physician.

Monitor and document patient conditions, including vital signs, wound care, drains, and lab values.

Assist with medical procedures, treatments, and medication administration as per scope of practice.

Collect and document patient data using specialized equipment as needed (e.g., Maintain a clean, safe, and therapeutic environment following safety and infection control protocols.

Communication & Collaboration Establish and maintain communication with the healthcare team, patients, and families.

Inform the supervising Registered Nurse/Physician about patient activities, observations, and changes in condition.

Provide patient and family education related to health conditions, treatment plans, and home care.

Documentation & Compliance Ensure accurate and confidential maintenance of patient medical records.

Follow Emory Healthcare's policies, procedures, and regulatory requirements.

Quality Improvement & Professional Development Participate in quality improvement initiatives and adhere to best practices in patient care.

Conduct or assist with in-service training and continuous learning opportunities.

May serve on sectional, departmental, or organizational committees as needed.

Minimum Qualifications Education - High School Diploma or GED required.

Graduate of an accredited Licensed Practical Nursing Program.

Licensure - A valid, unencumbered license as a Practical Nurse in the State of Georgia.

If completing virtual care activities that may include multi-state practice, an active compact/multistate license (eNLC) is required within 60 days of hire. Skills: Strong patient care, communication, and documentation skills.

Preferred Qualifications Experience: Previous nursing experience preferred but not required.

Physical & Environmental Requirements PHYSICAL REQUIREMENTS: (20-35 lbs, 34-66% of the workday; (10-20 lbs, 67-100% of the workday (constantly); Close eye work (computers, typing, reading, writing); ENVIRONMENTAL FACTORS: Factors affecting environmental conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Chemicals/gases/fumes/vapors; Electrical shock; Floor Surfaces; Indoor/Outdoor conditions; Patient care/handling injuries; Shift work; Travel may be required; Use of personal protective equipment, including respirators; environmental conditions may vary depending on assigned work area and work tasks.
