RN - Registered Nurse Ambulatory Care -Full Time

Nurse Navigator Job Description The Nurse Navigator assists in coordinating care for patients with cardiac rhythm disorders and serves as a key resource between patients, cardiology providers, electrophysiologists, clinical staff, Cath Lab staff, and scheduling teams. As a single point of contact, the Nurse Navigator will provide medical information, education, and share knowledge regarding internal and external support services and resources.
Key Responsibilities Coordinate care including consultations, follow-up visits, and procedures and provide timely follow-up from procedures.
Assist with monitoring message pools and communicate with patients regarding medication changes, pre-procedure planning, and follow-up post procedures.
Coordinate scheduling for procedures with clinic scheduler, anesthesia, imaging, etc. Escalate urgent clinical concerns to the appropriate provider for review.
Cardiology
Hours: Full-time/40 hours per week
RN; Pay: Based on experience, starting at $35.43 per hour
Initiates and documents patient teaching, including family and significant others, based on assessment of needs.
Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, office staff, insurers, and facilities.
Schedules patients for diagnostic, therapeutic, referral, or additional office visits.
Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures.
Assists in coordination of end-of-life care for patient and provides emotional support.
