Clinical Operations

Shift: M-F

Hours per Shift: 8:00 AM - 4:30 PM

Experience: Minimum of one year of nursing experience in a clinical setting with a frail or elderly population.

Unencumbered RN License

Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.

Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.

Ability to work independently with minimal supervision.

Under the direction of the Primary Care Provider and supervision of the Clinic Supervisor, the Clinic Nurse is responsible for providing nursing care to participants in the clinic as assigned.This includes support with clinic overflow, checking vital signs, performing POC testing, managing and administering medications, and supporting wound care and any other clinical needs in accordance with the scope of practice.The Clinic Nurse will manage ongoing care and complex clinical situations, assist with procedures as needed, and provide clinical support to ensure efficient operations of the clinic.Perform and document ordered procedures and care in the clinic, including medication management and administration in accordance with the scope of practice, as directed by the Primary Care Provider.Remain alert to input from team members, participants, and caregivers, and update the RN Case Manager and/or other clinic staff promptly of any changes in participant conditions.Support clinic staff to ensure smooth care transitions between settings such as hospitals, skilled nursing facilities, and home.Dispense ordered medications to participants while in the clinic and reinforce education regarding medications.Instruct and/or provide as-needed teaching to participants, family, and caregivers regarding disease processes, self-care techniques, and prevention strategies.Provide backup coverage to other nursing functions as assigned and perform PGX testing in accordance with the scope of practice.Process Specialty Pharmacy medication orders and elevate to MD as needed while addressing participant medication issues and questions in the clinic.Perform pre-enrollment medication evaluation and provide coverage as needed to support the care team in delivering patient care.

Administrative Responsibilities:Maintain clinical trackers such as TB screening and vaccinations.Monitor and maintain Emergency Cart logs and restock as needed.Stock and manage medication supplies in accordance with policy.
