DIETITIAN - DIETICIANS ( FULL TIME)

Hampton Court Nursing and Rehabilitation Center

We are seeking a compassionate, knowledgeable, and motivated Registered Dietitian to join our team. The ideal candidate will assess patients' nutritional needs, develop individualized nutrition care plans, provide nutrition education and counseling, and collaborate with interdisciplinary healthcare professionals to support optimal health outcomes.

Key Responsibilities

Conduct comprehensive nutrition assessments for patients.

Develop, implement, and monitor individualized nutrition care plans.

Provide medical nutrition therapy and nutrition counseling.

Educate patients, families, and caregivers on healthy eating habits and disease-specific nutrition.

Monitor patient progress and adjust nutrition interventions as needed.

Maintain accurate and timely documentation in the electronic health record.

Collaborate with physicians, nurses, therapists, and other healthcare professionals.

Participate in quality improvement initiatives and maintain compliance with regulatory standards.

Stay current with evidence-based nutrition practices and continuing education requirements.

Supervise the dietary department staff, ensuring compliance with safety regulations and quality standards.

Collaborate with healthcare teams in hospitals to integrate dietary considerations into overall patient care Offer health coaching sessions to support patients in making sustainable lifestyle changes.

Stay updated on the latest research in dietetics, particularly related to geriatrics and tube feeding practices.

Qualifications

Bachelor's or Master's degree in Nutrition, Dietetics, or a related field.

Current state licensure/certification, if applicable.

Strong communication, counseling, and interpersonal skills.

Excellent organizational and documentation skills.

Ability to work independently and as part of a multidisciplinary team.

Experience in clinical, long-term care, outpatient, or community nutrition is preferred.
