Physical Therapist – Home Health

Estimated Weekly Gross Pay: $2,600–$2,700/week
Contract Length: 13 Weeks
Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment with CenterWell Home Health in Plano, Texas. This opportunity allows you to provide individualized, one-on-one care that helps patients regain mobility, improve independence, and safely remain in their homes while working with a collaborative interdisciplinary care team.
Perform comprehensive physical therapy evaluations in patients' homes.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers.
Complete timely and accurate documentation using Homecare Homebase (HCHB) .
Travel throughout the assigned coverage area while delivering exceptional patient-centered care.
Active Texas Physical Therapist license (or ability to obtain before start)
Minimum of 1 year of recent Home Health experience
Homecare Homebase (HCHB) experience preferred
Valid driver's license, reliable transportation, and current auto insurance
