This role is responsible for conducting comprehensive assessments, developing individualized care plans, and ensuring patients receive high-quality, personalized care while promoting independence, safety, and overall well-being.**Positions Available*** Weekend Admissions Nurse* Weekday Admissions Nurse**Duties/Responsibilities:*** Complete home health admissions, comprehensive patient assessments, and ongoing evaluations.* Develop and implement physician-ordered plans of care while accurately completing all required documentation, including OASIS assessments.* Provide skilled in-home nursing care to multiple patients throughout the day.* Educate patients and caregivers on medications, disease processes, treatment plans, and in-home care needs.* Communicate changes in patient condition to physicians and interdisciplinary care team members.* Ensure compliance with federal, state, and agency regulations, policies, and standards of care.* Collaborate with the interdisciplinary team to promote positive patient outcomes and continuity of care.**Physical Requirements*** Ability to sit, stand, and drive for extended periods while providing in-home care.* Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.* Regular use of a tablet for documentation, communication, and training.**Disclaimer:***This description reflects the general nature and level of work for the Admission Registered Nurse Position.
