MDS Coordinator Position Summary

Attend weekly education meetings to stay updated on MDS changes.

Coordinate the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.

Accurately complete all MDS assessments and any supporting assessments or clinical documentation.

Evaluation of resident’s comprehensive plan of care, auditing medical records for supporting documentation, collaborating with the interdisciplinary team.

Perform any additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.

Maintain confidentiality of protected health information, including verbal, written and electronic communications.

MDS Coordinator Requirements

Active LPN or RN license for the state of Tennessee

2-5 Years Experience with PDPM, CMI, and Quality Measures

RAC Certification preferred
