Physical Therapist-Health coverage

Pride Health is hiring a Physical Therapist to support our client's home health program in The Villages and surrounding areas, FL (Coverage Areas: Epping Care territory). This is a 3 Months Contract (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Job Title: Home Health Physical Therapist
Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Job Type: Local contract
Contract Length: 3+ months
Pay offered is based on experience, expertise, credentialing, and education.

On-Call/Weekend Requirement: Yes
Mileage Reimbursement: Yes (between patient visits)
Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks
Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence
Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education
Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides
Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team
Participate in case conferences, care coordination activities, and team collaboration
Ensure compliance with CMS regulations, infection control standards, and agency policies
Adapt quickly to new environments, workflows, and patient populations as a travel clinician
Graduate of an accredited Physical Therapy program
Current Physical Therapist license in the state of assignment (Florida, if applicable).
Minimum of 1 year of clinical experience as a Physical Therapist (home health preferred)
Experience treating older adults and medically complex patients
Valid driver's license, reliable transportation, and active auto insurance
Willingness to travel within the assigned coverage area
Active Physical Therapist license in the state of assignment
