These are the workers who have retired from Department of Energy facilities such as Lawrence Livermore National Lab, Sandia National Lab, Lawrence Berkeley National Lab, Stanford Linear, etc. We offer flexible schedules, competitive pay, and 24/7 available support.Unlike many home health agencies that require multiple short visits per shift, Hallway Healthcare typically assigns you to one patient per shift, allowing for more focused, consistent, and meaningful care.Palliative CareIV Therapy (certified)Diabetes CareAlzheimer's and DementiaOstomy/Colostomy CareWound CareNasopharyngeal/Tracheostomy Care (limited)Catheter CareMedical GasesObservation and Assessment related to patient's medical challenges and symptom fluctuationsTeaching and Training ActivitiesRequired Education, License and Certification:Graduated from an accredited school of professional nursing.Current California RN/LVN LicenseCurrent BLS CertificateValid Drivers LicenseResponsibilities / Essential Functions:Demonstrate effective teamwork and collaboration with staff members.Support and contribute to quality improvement initiatives.Perform nursing procedures in accordance with agency policies and procedures.Work under the supervision and direction of a registered nurse (RN).Educate patients and their families about disease processes, medications, plans of care, treatment options, and home care procedures as outlined in the plan of care.Report any adverse findings or concerns to the physician and RN promptly.Assist with activities of daily living, such as bathing, dressing, feeding and light housekeeping.Transport patients to doctors appointments, grocery stores or pharmacies.Coordinate and monitor patient care services to ensure optimal outcomes.Follow infection control policies rigorously in all settings.Document skilled visits accurately and in accordance with established guidelines. Documentation must be completed by the end of each shift. The role involves managing challenging interactions with staff, public officials and private representatives while interpreting and implementing departmental policies and procedures.Employee will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client's home environment. Some homes will be clean, neat, and maintained at a comfortable temperature.
