Clinical Liaison (Care Transitions Coordinator)

As a Clinical Liaison (Care Transitions Coordinator), you will assist patients with the process of navigating post-acute care. Serves as the primary relationship manager between the company and referral sources including physicians, physician practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations or progressional. Supports referral growth by providing awareness and education of company services and program requirements to referral sources, patients, families, caregivers and community partners to help navigate post-acute care transitions with the goal of improving patient outcomes, supporting care transition, and promoting exceptional quality and patient / referral source satisfaction.

Required to have Bachelor's Degree in, healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work.

Highly preferred graduate of an approved school of nursing, therapy or social work.

Required minimum two years of experience in healthcare business development, community relations, marketing, or sales.

Required demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.

Preferred established relationships with physicians, hospitals or senior living communities and other referral sources.

Oral communication, written communication, fluency in English, active listening.

Especially Microsoft office, including word and excel.

Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
