Licensed Vocational Nurse (LVN) Visiting Home Health

The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in a home care setting to provide nursing services.

Providing nurse visits to home health patients.

Care for ill, injured, or convalescing patients by implementing plan of care initiated by RN.

Provide basic patient care and treatment, and document consistently with the plan of care.

Educating the patient how to take care during non-visit days.

Demonstrate knowledge of regulatory requirements.

Our patients need care 7 days a week. Though you have flexibility in your scheduling, we do ask that you are able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

Visiting LVN for Home Health: 1 year (Required)

Nursing: 1 year (Required)

Current CA LVN License (Required)

NSO Nursing License Insurance (Required)

IV/Phlebotomy Certified (Preferred)

W-2 Per diem
