Physical Therapist - PT - Full Time - T

There is currently an urgent need for a licensed PT to join a premier, highly-respected health care organization on a full-time basis. This role would travel to assisted living facilities and patient homes to provide treatment. The territory is a 50 mile radius around the Fond du Lac Area and no overnight travel would be needed.
Job Description Participate in the development of patient physical therapy plans of care
Make visits to patients for assessment/evaluations and communicate with physicians before start of care and as needed
Implement the plans of care and evaluate outcomes
Document patient care
Provide therapeutic treatments and evaluation of equipment need as applicable.
