$Perform, assist, evaluate and document medical care, including basic first aid, taking weight and vital signs, catheter and bowel care with I/O monitoring, range‑of‑motion exercises, ambulation, transfer and positioning with wheelchairs and other medical equipment, and preparing meals according to dietary needs.

Care for patients’ special conditions and needs within the scope of authorized tasks.

Provide hands‑on personal care and simple procedures as part of therapy or nursing services.

Perform services ordered by the physician in the plan of care that the aide is permitted to do under state law.

Assist patients in maintaining good personal hygiene, including bathing, toileting, grooming, shaving, skin and nail care, oral hygiene, dressing and eating.

Observe and report significant changes in a patient’s condition to the RN Nursing Supervisor.

High school graduate or GED*

Current, unrestricted certification or proof of training as required by the state.

Demonstrated proficiency in clinical assessments, documentation, and compliance with aide care policies and procedures.

The Aide must receive at least 12 hours of in‑service training during each 12‑month period.

Speak, write, read and understand English.

Travel and lift up to 50 pounds.

Visual and hearing acuity; Reposition patients and move equipment without assistance.

Respond physically and mentally to emergency situations.

Possible exposure to blood, bodily fluids and infectious diseases.
