**Full Time Schedules
- Some weekends may be required
- 8 or 12 hour shifts available depending on the facility
- Contract Position (flexible with shift availability is a plus)
- Conduct crisis intervention and risk assessments
- Connect clients with community resources and support services
- Monitor client progress and adjust care plans as needed
- Maintain accurate clinical documentation and case records
- Collaborate with healthcare providers and community agencies
- Advocate for clients to improve access to care and support
**Graduation from an accredited educational institution
- Licensed as a Licensed Clinical Social Worker by the State in CA. American Heart Association BLS, for Heath Care Providers
- Licensed master social worker (LMSW or LCSW).
- EPIC experience (preferred)
- Health, Dental, Vision, Life, and Short-Term Disability Insurance
