Clinical Care CoordinatorWe are looking for a highly experienced Clinical Care Coordinator. Candidates should have experience in acute inpatient case management and discharge planning. Training on the unit will be minimal, and candidates must be able to pick up quickly and work efficiently. The facility reserves beds for complex placements, but specific criteria must be met for assistance. Candidates will receive a variety of complex patients due to the facility's status as a trauma hospital. This position requires rotational holiday coverage as needed.
