Position: Full-Time Field Registered Nurse (RN)

At Home Care Partners is seeking a compassionate and experienced Home Care Field Registered Nurse (RN) to provide clinical care and support to patients throughout Albany County, New York .
This full-time opportunity is ideal for an RN who enjoys independence, one-on-one patient interaction, and the opportunity to make a meaningful impact outside of a traditional hospital or facility environment.
As a Field RN , you will travel to patient homes and community-based settings throughout Albany County to complete nursing assessments, oversee individualized Plans of Care, supervise home health aides, and help ensure patients receive safe, consistent, and personalized care.
Travel to patient homes and community-based settings throughout Albany County
Conduct comprehensive in-home nursing assessments
Develop, review, and update individualized Plans of Care (POC)
Complete required home health aide supervisions
Orient aides to each patient's Plan of Care and specific care needs
Collaborate with patients, families, caregivers, aides, and office staff
Help maintain continuity, safety, and quality of patient care
Maintain compliance with applicable healthcare regulations and company policies
Current and active New York State Registered Nurse (RN) license
Ability and willingness to travel throughout Albany County and assigned service areas
Medical Professional Liability insurance
Comfortable using Gmail and Microsoft Word/Excel
Ability to work independently and effectively manage a field-based schedule
Previous home care or home health nursing experience
Community health or outpatient nursing experience
Conducting in-home nursing assessments
Completing aide supervisions
Developing, reviewing, or updating Plans of Care
Bilingual Spanish/English is a plus
