Title: Physical Therapist

Duration: 3 months contract + Possibility of Extension

Location: New Braunfels, TX

Shift: 8 AM - 4:30 PM

Coverage Areas: 60 mile radius

About the Role We are seeking a skilled and compassionate Travel Physical Therapist to deliver high‑quality care to patients in a home health setting. In this field‑based role, you will provide individualized therapy services to help patients restore mobility, improve function, and enhance quality of life—right in the comfort of their homes. This position is ideal for clinicians who are independent, adaptable, and passionate about patient‑centered care.

Roles & Responsibilities Provide skilled physical therapy services to patients in their homes in accordance with physician orders and care plans.

Evaluate patient conditions, develop treatment plans, and set functional goals.

Implement therapeutic interventions to improve mobility, strength, balance, and overall function.

Educate patients and caregivers on exercises, safety practices, and ongoing care plans.

Monitor patient progress and adjust treatment plans as necessary.

Document all patient care activities accurately and in a timely manner.

Collaborate with interdisciplinary care teams to ensure coordinated and effective treatment.

Maintain compliance with clinical guidelines, agency policies, and regulatory standards.

Travel to patient homes within assigned coverage areas (up to a 60‑mile radius).

Minimum Qualifications Active Physical Therapist (PT) license.

Bachelor’s degree (or higher) in Physical Therapy.

Ability to deliver skilled care independently in a home health setting.

Strong clinical judgment and patient assessment skills.

Preferred Qualifications Previous home health experience.

Experience with HCHB (HomeCare Homebase) or similar EMR systems.

Strong time management and organizational skills.

Excellent communication and patient education abilities.

Flexibility and adaptability in a field‑based environment.

Work Environment & Requirements Field‑based role requiring travel to patient homes. Must be able to perform physical therapy interventions, including patient mobility assistance, and have reliable transportation required for travel within the assigned radius.
